Supportive housing is the primary component of the Cardinal Housing Network program. This first property will house 10 single women in a group home setting. This property is best suited for individuals seeking accountability in a supportive, community centric environment. This property is available through community partner referral only. Acceptance into the program is determined on a case-by-case basis.
There are 5 bedrooms, 3 bathrooms, a full kitchen, and separate kitchenette and laundry facility in the basement. Each resident will have access to a personal storage cabinet to lock belongings. Residents will be encouraged to stay a minimum of 90 days and be actively engaged in self betterment. The maximum length of stay at this property is 2 years. Residents who would like to extend their housing at this property must submit a letter to the CHN Board of Directors and will be approved on a case by case basis. This ensures potential residents new to recovery have the opportunity to utilize the extensive services provided at this property. This property has a security camera system in common areas to support correspondence to officers supporting court ordered placement
Cardinal Housing Network will utilize the expertise of community partners to provide case management services. Case management services may include but not limited to: (1) mental health and medication management and scheduling, (2) budgeting support (3) court ordered requirements, (4) medical appointment scheduling, (5) obtaining government documents among others.
The referral partner’s role in case management is to develop, coordinate, and communicate with CHN regarding the potential resident’s case plan. Cardinal Housing Network will rely on referral agencies, independently or in coordination with other Douglas County based service providers, to provide case management services listed above for referred residents during their stay with CHN for a minimum of 12 months.
CHN recognizes that not all potential residents may require on-going case management services. The referring agency will need to provide sufficient background information on the potential resident’s completion of a case plan and agree to serve as a reference for a minimum of 12 months upon potential resident’s admission. Cardinal Housing Network does not hold service provider preferences, and encourages residents to explore services that best suit their needs.
Cardinal Housing Network’s role in providing supplemental case management is to support the case plan put in place by the referring partner. Our responsibilities will include:
- Community connections, including group participation in local SUD or supportive networks 
- Communication with referral partners and service providers to support advancement of case plan 
- One-on-one check-ins between resident and CHN Support Coordinator for advancement of case plan 
- Providing structured, community centric housing 
- Educational programming for resident enrichment 
- Completing in-take and data tracking services for community reporting 
- CHN and referring agency will coordinate safe transportation 
- Residents will be given a tour of the house and assigned a bed based on physical accommodation needs. Residents requiring ADA accessibility will be given bed assignment priority. CHN will provide accommodations for residents with limited mobility (laundry services) 
- Residents will meet with CHN Support Coordinator to complete relevant in-take forms 2 - Include all services they are receiving from government or non-profit agencies 
- Include organizations they are in community with (churches, peer groups, NA/AA chapters) 
- Disclosure of case managers and service providers, including contact information and release of information 
- Disclosure of emergency contact(s) 
- Visitation accommodations needed with children 
 
- Residents will sign acknowledgement of non-negotiable offenses 
- Identify what government documentation is needed and work with case manager to obtain 
- Identify need and/or apply for governmental assistance with case manager (food stamps, healthcare) 
- Identify community service requirements to complete court-ordered rehabilitation 
- Personal finance obligations and debts (court fees, fines, child support, etc.) 
- Budget development (payment plans, income needs) 
- Identify employment opportunities or enrollment dates for programs/schooling. Full time employment is categorized in CHN as 30 hours minimum per week. 
- Introduction of S.M.A.R.T. goal setting 
Cardinal Housing Network will work to incorporate programming that assists in personal development and spiritual growth. These programming efforts will be housed primarily at 1046 New Hampshire St. or in South Park, the common community space between all three properties.
Programming may include:
- In-home outpatient treatment or in-home training seminars by local SUD partners 
- Attendance to broader community events (Art in the Park, parades, community service) 
- Monthly coaching seminars paired with group activity (art, cooking, baking, gardening, life skills, home repair) 
- Community building activities (Quarterly group outings) 
- Physical wellness and activity (Trauma informed yoga, somatic exercises, walking groups, etc.) 
- Program fees will be $125/week. This includes rent, utilities, supplies. 
- Promise to Pay note required by referring agency prior to admission 
- Program fees are billed every Monday 
- If a resident falls $300 or more behind on program fees, the resident will be placed back on contract. Balance must be $0 to be removed from the contract. 
- Residents should talk to CHN staff if income circumstances change. A payment plan can be arranged on a case-by-case basis. 
- Program fees are subject to change with CHN Board of Director vote
