Program Fees at 1046 New Hampshire St.Program fees will be $125/week. This includes rent, utilities, supplies. Supplemental Case Management ServicesCardinal Housing Network will utilize the expertise of community partners to provide case management services. Case management services may include but not limited to: (1) mental health and medication management and scheduling, (2) budgeting support (3) court ordered requirements, (4) medical appointment scheduling, (5) obtaining government documents among others. The referral partner’s role in case management is to develop, coordinate, and communicate with CHN regarding the potential resident’s case plan. Cardinal Housing Network will rely on referral agencies, independently or in coordination with other Douglas County based service providers, to provide case management services listed above for referred residents during their stay with CHN for a minimum of 12 months. CHN recognizes that not all potential residents may require on-going case management services. The referring agency will need to provide sufficient background information on the potential resident’s completion of a case plan and agree to serve as a reference for a minimum of 12 months upon potential resident’s admission. Cardinal Housing Network does not hold service provider preferences, and encourages residents to explore services that best suit their needs.

ProgrammingCardinal Housing Network will work to incorporate programming that assists in personal development and spiritual growth. These programming efforts will be housed primarily at 1046 New Hampshire St. or in South Park, the common community space between all three properties. Programming may include: In-home outpatient treatment or in-home training seminars by local SUD partners Attendance to broader community events (Art in the Park, parades, community service) Monthly coaching seminars paired with group activity (art, cooking, baking, gardening, life skills, home repair) Community building activities (Quarterly group outings) Physical wellness and activity (Trauma informed yoga, somatic exercises, walking groups, etc.)

HousingSupportive housing is the primary component of the Cardinal Housing Network program. This first property will house 10 single women in a group home setting. This property is best suited for individuals seeking accountability in a supportive, community centric environment. This property is available through community partner referral only. Acceptance into the program is determined on a case-by-case basis. There are 5 bedrooms, 3 bathrooms, a full kitchen, and separate kitchenette and laundry facility in the basement. Each resident will have access to a personal storage cabinet to lock belongings. Residents will be encouraged to stay a minimum of 90 days and be actively engaged in self betterment. The maximum length of stay at this property is 2 years. Residents who would like to extend their housing at this property must submit a letter to the CHN Board of Directors and will be approved on a case by case basis. This ensures potential residents new to recovery have the opportunity to utilize the extensive services provided at this property. This property has a security camera system in common areas to support correspondence to officers supporting court ordered placement

Referral Guidelines to Group HomeThis is an introductory house with the most intensive services and monitoring. This property is ADA accessible and equipped to accommodate individuals with limited mobility. Be referred by a DGCO social service partner familiar with the potential residents history. Preference will be given to DGCO residents and DGCO based social service providers. Must be 18 years or older Must identify as female or femme, trans and non-binary residents welcome Must have completed a period of sobriety before admission: This can include in-person rehabilitation, outpatient rehabilitation, detox, imprisonment, or hospital admission Residents must be medically stable (not actively experiencing withdrawal symptoms) CHN does not have the licensure to accommodate individuals experiencing a health crisis Individuals in a health crisis will be referred to Lawrence Memorial Hospital (LMH) or the Treatment and Recovery Center (TRC) Potential residents should indicate intent to remain sober to the referring agency. Factors to consider are motivation and on-going participation in recovery community activities (out-patient treatment, AA/NA support groups, additional support groups etc.). Agree to House Rules and 30-day probationary period upon in-take Complete in-take forms and relapse plan with CHN Support Coordinator Be notified of safety surveillance in common areas of property (entrances, living room, kitchen, basement). No audio is recorded. NO surveillance in private spaces (bedrooms, bathrooms) Be notified of $125/weekly program fees (includes rent, utilities, supplies, evening meals) Be notified that employment or enrollment in an educational program must be obtained within 60 days of admission Acknowledge that residents may stay up to 2 years at this location. New Resident 30-Day Probationary PeriodAll appointments within the first 30 days need to be communicated to and approved by CHN Support Coordinator Participants cannot be away from the house except for approved appointments, peer training, job interviews, treatment, or legal obligations. Must attend mandatory house meetings on Sundays and Wednesdays at 6pm and CHN community activities unless prior approval has been obtained by CHN Support Coordinator No day or overnight passes All phone calls during this period should be made to immediate family members, sponsors, legal representation, or care coordination team No visitation is allowed during the probationary period unless court ordered by DFS guidelines for children Evening curfew is at 10pm Failure to follow any of these rules will result in an extended probationary period and initiate communication with the case manager on how to best address behavioral concerns. Infractions in the probationary period are NOT grounds for dismissal unless noted in the non-negotiable agreement. For residents to be graduate from the probationary period: Be a program participant for a minimum 30 days with no major rule violations Must be actively engaged in self betterment. They must begin exploring employment, enrollment in school, and/or personal development classes. Have established a financial plan to pay program fees and other budgetary needs Must have attended all mandatory house meetings and be actively engaged with community support groups

Contract ResidentResidents may be placed on contract that may result in revocation of full resident privileges. The contract is crafted between CHN Support Coordinator and is intended to provide additional accountability. Contracts will be developed on a case-by-base basis. Residents may be placed on contract if: Resident has accumulated 3 written infractions Resident has not attended mandatory house meetings Resident has fallen more than $300 behind in program fee payments Resident has caused disruption in the broader CHN community, identified in written infractions All written infractions are kept in the resident’s file and a copy is shared with the resident. Contracts will require a signature of adoption between CHN staff and the resident.

ChoresSafety is our primary concern. These rules have been crafted to create a safe, supportive environment where both community and individual healing can occur. These rules are applicable to all probationary, full, and contract residents. Chores will be assigned by the CHN House Manager on Sunday evenings. Each resident will be assigned at least one chore per week. Chore responsibilities will be posted on a white board. Residents should initial and date their completed chores as soon as completed. CHN House Manager will initial the chore chart upon review of chore completion. Non-compliance of chore responsibility will result in written infraction.

These rules are applicable to all probationary, full, and contract residents. Visitation (one time or on-going) involving resident’s children should be communicated with CHN upon admission. Residents should provide CHN Support Coordinator 3 day advance notice of changes to regular visitations to ensure accommodations are met Any visits from service providers must be approved through CHN Support Coordinator. Significant others and family friends are not allowed to visit the property for any reason Any unauthorized visitors will result in a written infraction

Community ExpectationsSafety is our primary concern. These rules have been crafted to create a safe, supportive environment where both community and individual healing can occur. These rules are applicable to all probationary, full, and contract residents. CHN Support Coordinator and CHN Program Director are the only authorized persons to approve passes. CHN Live-in House Manager is not authorized to approve passes. There are three classifications of residents: probationary, full, and contract. All residents are encouraged to be active members of their housing community All residents must attend weekly meetings on Sunday & Wednesdays at 6pm Chores are assigned by the House Manager on Sunday evenings and must be completed on schedule All residents will be required to sign in/out before returning/leaving the house All residents must submit to urine analysis or breathalyzer when asked to do so. Residents will have one hour to comply. Failure to do so will be considered a positive and will result in dismissal from the program. All medications must be disclosed upon admission. Prescriptions and over the counter medications will be kept in a locked safe. Please see the ‘Medication Management’ policy below. No food is allowed in bedrooms at any time All residents will be expected to honor quiet hours between 11pm-7am All residents will be expected to change and launder their bed linens once a week All residents will be expected to make their bed every morning All clothes should be kept in drawers or in closets Turn off fans, lights, and all electrical devices when not in the room Please report all damage to the house or furniture immediately No food is allowed in bedrooms at any time No guns, knives, or weapons of any kind are allowed in the house

Abandonment/Termination of Resident PolicySafety is our primary concern. These rules have been crafted to create a safe, supportive environment where both community and individual healing can occur. These rules are applicable to all probationary, full, and contract residents. All non-negotiables have been identified as potential threats to the greater safety and wellbeing of all CHN residents Non-negotiables have been divided into two categories: High Threat and Medium Threat If the violation is a violent offense, CHN will enact High Threat precautions and seek support from local law enforcement to diffuse the situation and escort resident off property Residents who have been asked to leave the program for Medium Threat violations will be given until the end of business day (5pm) on the date of notification of dismissal to move out. This grace period allows the resident and CHN time to coordinate other housing arrangements so as to not place our CHN family members in potentially dangerous situations If a Medium Threat violation takes place after business hours, CHN staff will address the violation within 1 hour of notification and provide supportive staffing and alternate sleeping arrangements for the evening (basement cot) If a violation takes place after business hours, the resident will be asked to move out the following day by 10am. Residents departing the program must take all of their belongings with them when they move out. If a resident is unable to take items due to a medical emergency, the resident must arrange for property retrieval with the CHN Support Coordinator. Arrangements must be made within 5 days of departure and MUST be picked up by the resident. If arrangements are not made within 5 days, all property is deemed abandoned. Cardinal Housing Network will donate items to a local non-profit. The organization does not assume responsibility for abandoned items.

Fire SafetySafety is our primary concern. These rules have been crafted to create a safe, supportive environment where both community and individual healing can occur. These rules are applicable to all probationary, full, and contract residents. Fire extinguishers and fire suppression blankets can be found at the following locations: (1) Entry way, (2) foyer on the second floor, (3) basement, (4) under sink The property is equipped with a fire suppression system that will automatically activate in the event of a fire. Where are these located? If you discover a fire, call 911 immediately If possible and safe to do so, alert peers of fire and need to evacuate Close all doors behind you Use both exits to evacuate quickly Report to assembly point in front yard There are to be no candles or flames allowed anywhere in the house at any time.

Medication ManagementSafety is our primary concern. These rules have been crafted to create a safe, supportive environment where both community and individual healing can occur. These rules are applicable to all probationary, full, and contract residents. All medications MUST be disclosed upon in-take, including frequency & preferred administration time Residents will receive a designated 3x/day allotted pill distributor and lockable safety canister All non-sorted medications will be securely stored in a locked safe, accessible only by CHN staff Residents and CHN staff will maintain a weekly pill log Any changes to medication lists must be disclosed with CHN Support Coordinator immediately

LaundrySafety is our primary concern. These rules have been crafted to create a safe, supportive environment where both community and individual healing can occur. These rules are applicable to all probationary, full, and contract residents. The laundry equipment is shared by all residents in the house. There are two washers, two dryers, and a wash basin for your use. CHN Staff will show you the laundry area upon intake and show you how to use the machines. Residents needing ADA accommodations can request laundry services provided by CHN Staff Residents are required to wash their own clothing and bedding Laundry must be done between 7am-9pm Laundry must not be left unattended in the washer or dryers. Laundry must be promptly moved between loads and never left in the machine. Please clean the dryer lint trap after every use. Do not overload the washer and dryer. Bath towels, sheets, blankets, hand towels, and washcloths should be washed weekly.

Respect for Greater CommunitySafety is our primary concern. These rules have been crafted to create a safe, supportive environment where both community and individual healing can occur. These rules are applicable to all probationary, full, and contract residents. Confidentiality must be maintained at all times (please see confidentiality policy below) Grievances should be addressed in accordance with grievance policy (see below) Entry into a bedroom without permission is strictly limited to the assigned resident of that room or a staff member of CHN Residents are responsible for their own room (locking windows, keeping room tidy, etc.) Respect for housemates by not using abusive language or gesture No gossiping No bullying, including forms of intimidation or verbal abuse that can cause emotional harm No food or beverages outside of the kitchen, dining room, and basement No excessive vulgar language There shall be no fraternization between program residents Curfew for all residents is 10pm unless residents are attending a qualified activity that has been approved by CHN Support Coordinator

Smoking and vaping are only allowed in designated areas Any and all forms of smoking and vaping are prohibited inside the home Smoke and vaping are prohibited on city sidewalks Vaping CBD is prohibited All cigarette butts must be placed in trash Any violations to these policies will be result in a written infraction

Residents are required to sign in and out of the property at all times Only full residents can request overnight passes Residents on a 30 day probationary period cannot request an overnight pass Residents placed on contract cannot request an overnight pass The request must be made to the CHN Support Coordinator at least 7 days in advance of the requested date of overnight passes UA and breathalyzers will be administered immediately upon return to the property for overnight passes Residents must reach out to CHN staff if in a compromising situation and need emergency services, including transportation

ConfidentialityBy federal law, Cardinal Housing Network cannot confirm or deny your presence at any residence within the network. All residents will be asked to provide a signed acknowledgement Residents will be asked to sign a Release of Information (ROI) allowing CHN staff to communicate with an emergency contact, family members, physicians, government agencies, partnering non-profits. Refusal to sign an ROI will limit the amount of services CHN can provide for the resident.

Drug and Alcohol ScreeningResidents will be asked to submit to random drug screenings and/or breathalyzers. Compliance with requested testing is mandatory and must be done within 1 hour of request. Failure to comply will be treated as a positive screening.

Notice of Privacy PracticesFederal law protects the confidentiality of substance use disorder (SUD) treatment records. For over 50 years, federal law has protected the privacy rights of people who seek treatment for SUD from federally assisted programs. The law and its implementing regulations are commonly referred to as 42 CFR Part 2. Confidentiality is a cornerstone of any treatment relationship. For people receiving SUD treatment, strict confidentiality protections mean that you can share information about past and current drug use without worrying that it will be used against you by the police, landlord, employer, judge, or social worker. For people considering whether to enter treatment, studies consistently show that confidentiality and privacy are major considerations. Until we have eliminated the stigma and criminalization of substance abuse disorders, patients need the right to access treatment without worrying that their records will not be used to take away their children, housing employment, insurance, public benefits, or freedom. 42 CFR Pt. 2 generally requires a patient’s written consent before making a disclosure of protected records. Patient consent must always be written and include specific information about the recipient of the records and the records to be shared. There are a few limited exceptions when providers can make disclosures without a patient’s written consent, including: Medical emergency Reports of alleged child abuse or neglect Reports of crime at Cardinal Housing Network residences Qualified audits or evaluations of the program Research Qualified service or organization agreement Part 2 generally requires a special court order before your records can be shared with law enforcement or a court. A subpoena, general court order, search warrant, or official request is not enough for law enforcement to access your treatment information. When Part 2 records are shared with a patient’s consent, the records must be accompanied by a notice prohibited redisclosure, so that the recipient knows they may not re-disclose information except as permitted by Part 2. Both Part 2 and HIPAA protect patient privacy by regulating the way that patient information can be shared and disclosed. HIPAA applies to many types of patient information, not just SUD information, and generally is less protective of patient privacy than part 2. Unlike HIPAA, Part 2’s privacy protections follow the records even after they are disclosed - the recipient of the records is now bound by Part 2’s privacy and security requirements for the Part 2 records.

Digital AccessUse of electronic media is a privilege that offers a wealth of information and resources. All searches on Cardinal Housing Network networks shall be educational in nature. Extreme caution must be taken to protect the sharing of personal information of the resident and the greater community. No sharing of passwords or log-ins with other residents. Residents shall not watch pornography or engage in pornographic endeavors on CHN wifi networks or devices. Use of CHN networks to engage in any criminal activity, including pirating content, is prohibited.

Consumer Rights and PrivilegesTo be evaluated and receive services in a clean and safe environment. To not be denied admission or services because of age, race, religion, gender, sexual orientation, creed, marital status, national origin, or disability. To confidentiality of information and records in accordance with state and federal law and regulations To be addressed by preferred gender pronoun To be free of verbal and physical abuse, neglect, corporal punishment, and another mistreatment such as humiliation, threats, and exploitation To consult with a private, licensed practitioner at one’s own expense To have private visits from a lawyer, doctor, or clergy person at reasonable times To correspond by sealed mail with government agencies and lawyers To humane care and to be treated with respect and dignity To have records be kept confidential

Grievance ProcedureFeedback and conflict resolution are crucial components in creating a safe, fair, and supportive recovery community. Any resident wishing to submit a grievance for any reason is asked to follow the procedure listed below. The following steps have been developed to resolve the grievance with community wellness at the core. Discuss the event with the CHN Support Coordinator. This should be done as soon as possible after the event. CHN Support Coordinator will seek to bring conflicting parties together in private conversation to find resolve. If no resolution is met, proceed to step two. If the grievance involves the CHN Support Coordinator, discuss the event with the CHN Live in House Manager first. The house manager will work with the Program Director to identify steps to address grievances. If the grievance involves the CHN live-in house manager, discuss the event with the CHN Support Coordinator first. Provide a written synopsis of the grievance and submit it to the CHN Support Coordinator and Program Director. Either the Support Coordinator or Program Director will provide a response to you within two (2) working days in writing. A scheduled time will be set to discuss the concern. CHN Staff will work to create mediation between conflicting parties. If necessary, a house meeting will be called to discuss and remedy grievances as a community. If a solution is not identified, the Program Director will work to accommodate affected resident(s) with the governing board if safety is compromised. The governing board will review this manual to clarify, revise, and/or implement policies and procedures as necessary Residents will not be reprimanded or made to suffer harassment or punishment from anyone in the Cardinal Housing Network for filing a grievance.

Photo ConsentAll residents give the Cardinal Housing Network consent to have any photos or videos published in any marketing or fundraising materials at the present time or in the future. Participants should not expect to seek compensation for the publication of their photos. Participants can choose not to give consent for photo or video publication and the organization will not publish photos or videos of a resident who does not consent. Residents not consenting are asked to not participate in photos or videos, and to notify staff prior to photos or videos being taken.

Good Neighbor PolicyThe Cardinal Housing Network, Inc. seeks to maintain a positive relationship with our neighbors and the community at large. If there are any problems, the House Manager and program director should be notified immediately to resolve the issue. Residents may not play loud music or engage in arguments outside of the houses. Residents with vehicles will be directed where to park to not disrupt neighborhood flow. The yard will be free of debris Lawn shall be maintained Smoking is allowed ONLY in designated areas. Smoking is prohibited on city sidewalks

Luggage Search and Personal PropertyLuggage, bags, room, and storage lockers are subject to search by CHN staff upon admission and during their stay at any given residence. The permission is to search all properties of the residents room. Items to be searched but are not limited to: Furniture Electronic Media Devices, such as cell phones, tablets, laptops, etc. Purses, wallets Backpacks, duffle bags, luggage, etc. As well as any or all of resident’s personal belongings Searches may be conducted for suspicious behavior or suspected fraternization
